Failures at the Passport Office; The Summer of 1999

In the summer of 1999 the passport agency and the United Kingdom faced a crisis. The implementation of the new computer system resulted in lengthy delays in the issuing of passports. This 10 page paper is written in two parts. The first part of the paper looks at the reasons why projects fail looking at a range of models that encompass a broad-spectrum of potential causes of failure.
